G. Renew Access or Submit Closeout

Access to controlled human data expires one year after the approval date. Prior to expiration, approved Data Requesters must submit a Project Renewal or Project Closeout.

Renew Access

Approved access may be renewed by submitting a Project Renewal prior to the expiration data. The renewal request includes a required Progress Update describing results and data use from the reporting period.

To submit your renewal request, complete the following form and send it to the DAC at nidcrfacebasedac AT mail DOT nih.gov:

Note: You will soon be able to submit a request for renewal from the online DAC process you used to process the original request.

Submit Closeout

When access to data is no longer needed, the Data Requester must submit a Project Close-out which describes any results from data use, as well as provides certification that any downloaded data has been destroyed in accordance with terms in the DUC.

To submit your closeout, complete the following form and send it to the DAC at nidcrfacebasedac AT mail DOT nih.gov:
